Form 1099-G reports the amount of refunds, credits and other offsets of state income tax during the previous year. The Department of Revenue’s 1099-G only applies to individuals who itemize their deductions on their federal income tax return. If you claimed the standard deduction on your federal return and did not itemize, you will not have a 1099-G. Note: Form 1099-G reporting an individual's unemployment amount is provided by the Department of Labor and Industrial Relations and is also available online.
1. The first social security number shown on your most recently filed return that you itemized.
2. The 5 digit zip code shown on your most recently filed return that you itemized.       Note: If you have filed a change of address with the United States Postal Service, you may       need to use the zip code of your new address. Taxpayers living outside of the United States       must enter 00000 in place of a zip code to obtain a copy.
